| Biological function: |
CircOGDH promotes ischemic neuronal injury and neuronal apoptosis, reduces neuronal viability under ischemic conditions, and serves as a predictive blood/exosomal biomarker of ischemic penumbra in AIS. |
| Molecular mechanism: |
CircOGDH acts as a miRNA sponge for miR-5112, upregulates COL4A4 expression, promotes neuronal apoptosis/injury under ischemic conditions, and is transported from ischemic brain tissue to peripheral blood through neuron-derived exosomes. |

| Clinical significance: |
CircOGDH is a highly specific and sensitive predictive biomarker for penumbra in patients with AIS; plasma CircOGDH showed an AUC of 0.9326 with sensitivity of 82.22% and specificity of 96.88%, and its expression was positively correlated with penumbra size. |
| Description: |
CircOGDH is up-regulated in ischemic penumbra tissue and in plasma/plasma exosomes of patients with acute ischemic stroke. It promotes ischemic neuronal apoptosis and injury by sponging miR-5112 to increase COL4A4 expression, while knockdown improves neuronal viability and reduces injury. Clinically, circulating/exosomal CircOGDH is proposed as a sensitive and specific predictive biomarker for the presence and size of penumbra in AIS. |
